A case of renal osteodystrophy with unexpected serum intact parathyroid hormone concentrations.
A 64-year-old female with long-standing end-stage renal disease (ESRD), status post 2 failed renal transplants, was evaluated for management of renal osteodystrophy with particular concern for adynamic bone disease (ABD). ABD was suspected because of low normal serum intact parathyroid hormone (PTH) concentrations (range 2.5–54 ng/L, reference range 10–65 ng/L), intermittently increased serum c...
متن کاملAre there any differences in the parathyroid response in the different types of renal osteodystrophy?
With the purpose of studying the curve of parathyroid response to variations of serum calcium during dialysis, we studied 20 patients on haemodialysis: 10 women and 10 men, with different forms of bone disease diagnosed by bone biopsy (adynamic bone disease, mild hyperparathyroidism, severe hyperparathyroidism). In all patients, we performed parathyroid stimulation by 4 h dialysis with 1 mEq/l ...
متن کاملRenal osteodystrophy.
The incidence of chronic renal disease is increasing, and the pattern of renal osteodystrophy seems to be shifting from the classic hyperparathyroid presentation to one of low bone turnover. Patients with persistent disease also live longer than previously and are more physically active. Thus, patients may experience trauma as a direct result of increased physical activity in a setting of weake...
متن کامل[Renal osteodystrophy and secondary hyperparathyroidism].
High-turnover bone due to excess PTH is one of the main types of renal osteodystrophy. In addition to the classic stimuli of PTH secretion such as hypocalcemia and decreased production of active vitamin D, new mechanisms has been suggested. Furthermore, skeletal resistance to PTH, a classic stimulus of PTH secretion in uremia, has been recently revisited as an underlying abnormality.
متن کاملSlipped epiphyses in renal osteodystrophy.
Clinical, biochemical, roentgenological, and histological features of slipped epiphyses (epiphysiolysis) in 11 out of 112 children with renal osteodystrophy have been analysed. Characteristic age-related patterns of involvement of different epiphyses are described. Quantitative measurements of iliac bone histology, serum parathyroid hormone levels, and clinical history show the presence of more...
